Licensed Clinical Social Worker – Member of OTSTCFQ
Services available in English and French across the province of Quebec

✏️ What the service includes:

•60 to 90-minute clinical assessment

•Official written psychosocial report in compliance with legal standards

•Coordination with professionals involved (e.g., physician, notary, lawyer)

•Secure delivery of the final report (encrypted PDF or via JaneApp)

•Insurance receipts and tax-deductible documents provided

💰 Fees:

•Standard Evaluation: $895 CAD (delivery within 7–14 business days)

•Express Evaluation: $1,195 CAD (delivery within 72 business hours)

•Additional fees may apply depending on complexity or special requests (bilingual report, family meeting, etc.)

📌 Important Notes:

•This service is recognized for use in court procedures, with a notary, or by the Curateur public du Québec.

•Some private insurance plans may cover part of the cost. Tax deductions may also apply.
